REMEDIAL OR RESTORATIVE TREATMENT
50% OF TREATMENT COSTS UP TO €400 A YEAR, FOR REMEDIAL OR RESTORATIVE TREAMENT.

ROUTINE EXAMINATIONS
Up to €80 a year for the cost of routine examinations

DENTAL X-RAYS
Up to €70 a year for the cost of dental x-rays

DENTAL ACCIDENTS
Up to €3,000 for each dental accident subject to a maximum of 4 claims a year up to a maximum of €12,000 a year.

EMERGENCY DENTAL TREATMENT
Up to €250 an incident subject to a maximum of 4 claims a year for the cost of emergency dental treatment within the Rep. of Ireland
Up to €500 an incident, subject to a maximum of 2 claims a year to a maximum of €1,000 a year, outside the Rep. of Ireland.

HOSPITAL CASH BENEFIT FOR DENTAL CARE AND TREATMENT
€75 a night, to a maximum of €1,000 a year, if you are admitted overnight as an in-patient to a hospital, under the care of a consultant specialising in dental or maxillo-facial surgery.

ORAL CANCER
Up to €12,000 a year for the cost of treatment of a primary cancer of the mouth and gums.

You will be subject to the Rules of Membership that are incorporated in the Membership Certificate and which will be sent to you immediately upon your election. This document contains a summary of cover only. Full details are contained in the Membership Certificate. You must be satisfied or you may return the HMCA Dental Plan within 30 days for a full refund.

EASY TO COLLECT
You submit your claim form, signed by the dental surgeon, plus your receipts.

ELIGIBILITY
You are eligible to become a member of this plan if you meet the following conditions:-
(i) You are permanent resident of Republic of Ireland
(ii) You are aged 0 to 78 years.

24 HOUR WORLD-WIDE COVER
You are covered anywhere in the world

EXCLUSIONS
These are kept to an essential minimum and are set out fully on the Membership Certificate

PRE-EXISTING CONDITIONS AND QUALIFYING PERIOD
You are not covered for any treatment which was planned or prescribed or is taking place at the commencement date of this plan, or for treatment within the qualifying periods which are:-

3 months for routine examinations, hygiene and x-rays
6 months for remedial or restorative treatment.
